	2010 NYC SCHOOL SURVEY Your School. Your Voice. Take your NYC School Survey by April 23! Take the survey online by clicking here or fill out the form (mailed directly to your home) and return it in the postage-paid envelope supplied to you. If you did not receive your survey form, please contact the NYC School Survey Team at surveys@schools.nyc.gov or call the Survey Help Line at 1-800-690-8603 or contact Jane Cascone, Parent Coordinator. The voices of parents, students, and teachers count in two important ways: survey results are reported to school leaders and the public through Survey Reports. The results also count for 10% of the school's yearly Progress Report grade. Our school's goal is to have 100% of our parents, students, and teachers to complete the survey. The first class to achieve 100% response rate from parents will receive a pizza party. The second class to reach the 100% goal will receive a dessert party. A chart depicting each class's response will be posted in the school cafeteria each week.
